All new clients must fill out, sign, and return both the T-REX RENTAL AGREEMENT and the T-REX CREDIT CARD AUTHORIZATION FORM before any equipment is booked.  

 

Please submit a picture of the front and back of the card with the Credit Card Authorization Form.

 

Additionally, a photocopy of a valid driver's license or passport for the person renting must be kept on file.
